Five Wishes® is a program made possible through Aging with Dignity. Five Wishes® helps you identify your desires in how you want to be treated if you get seriously ill.

We have received a grant from the Alaska Community Foundation
that allows us to provide a Five Wishes® pamphlet to you at no
charge.

We would also love to do a presentation for at your workplace,
place of worship, community group you belong to, etc.

Call the Hospice office @ 262.0453, or
